Service Description

Once this form is submitted you will receive an email from us with the link to the Cherry Application. Good for one Treatment, & 1 hour touchup. Additional hourly touchup fee is $100 and $50 each additional hour. Be sure to click on the Cherry Link option in our service menu to apply for your payment plan or fill out this form to receive a text to the Cherry Application Link to Finance your Brows, Soft Credit Pull, Rates apply. Service Pricing breakdown: $50 (deposit) $550 (full amount owed for treatment due at initial service) $150 (1 month mandatory touchup, 1 hour included at touchup appointment additional fees may apply) $750 for 120 minutes. (includes a 1 hour touchup rates apply for additional time) Fee does not include taxes or Cherry fees. Touchups are $100 first hour and $50 each additional hour. The 4 - 6 week touchup is mandatory. If you do not attend your included Touchup within 6 weeks time after the first appointment we cannot guarantee our work and you will be charged a $250 Color boost fee for your next service. Total service cost varies depending on each individuals needs. Any Discounts or promotions will be deducted at time of service. Non refundable if no call no show, you will be charged the full amount of service. Any touchups after 30 days are considered full price touchups. Any Booking fee’s are not applied to full price of services, Non refundable. PLEASE NOTE: that every client's skin will respond differently. Both in the length of time it takes to heal, and in the number of sessions it takes for your desired results and color retention. This information acts as a guideline only and we cannot guarantee results on anyone. Your results and longevity of your tattoo depends greatly on your aftercare during the healing process and beyond. How to get ready for the procedure? - Do not pick/tweeze/wax one week before procedure - Do not tan two weeks prior or have sunburned face - Do not have any type of facial/peel 2 weeks prior to treatment - Discontinue Vitamin A/Retinol products one month prior. - Do not work out the day of the procedure. - Do not have botox 3 weeks prior - Do not take Fish Oil or Vitamin E two weeks prior (natural blood thinners) - Do not wax or tint your eyebrows 3 days before the procedure. In order to avoid excessive bleeding and poor color deposit: - Do not drink alcohol 24 - 48 hours before your tattoo - Do not take an aspirin or ibuprofen for pain relief (this thins the blood)

Cancellation Policy

YOU WILL RECEIVE AN EMAIL CONTAINING YOUR CLIENT PAPERWORK AS SOON AS YOUR APPOINTMENT IS BOOKED, PLEASE PRINT AND FILL OUT ALL PAGES, BRING COMPLETED PACKET TO YOUR APPOINTMENT. PLEASE DO NOT RESPOND TO OUR EMAIL CORESPONDENTS, please call our studio if you need to cxl or change an appointment. Please Note, Any Additional Touchups are $150 first hour, $50 each additional hour. If your service includes a touch up you will forfeit the touch up if you: No Call No Show, Reschedule within 48 hours of your touch up date, or go past the 4 to 6 week touch up time frame. Cosmetic tattoo treatments are considered semi-permanent and will not wash off, however, they will fade over time and may need a touch-up every 6mos - 3 years. Brows, Lips,Liner or Other services Fee is only refundable if you are not eligible for services. Contraindications (ineligible)include: *Individuals under 18 years of age *Pregnant or nursing *Epilepsy *Pacemaker or major heart problems *Viral infections and/or diseases *LupusIn individuals with compromised immune system *Organ Transplant Recipients *Skin condition on or near the treatment area: Eczema, dermatitis, rosacea, psoriasis *Metal and Colorant allergy *Keloid Scarring *Accutane Users *Eligible With Doctor Clearance and Note Individuals undergoing chemotherapy A Doctors note is required in order to perform the procedure for anyone undergoing chemotherapy. Once you're cleared from your oncologist we can book your appointment immediately. *Diabetes It is important that your condition is stable and under control. Your physician will provide us with a doctor's note clearing you if under control. *Thyroid Condition The your brow color can fade faster when taken with certain thyroid medications. *Glaucoma * High blood pressure Individuals that have any uncertainty regarding their pre-existing conditions, or are under the supervision of a physician, please consult with your doctor before booking an appointment and provide us with a doctor's note. *Eligible Based on Individual Cases Individuals with a dark pre-existing eyebrow tattoo or permanent makeup The tattoo or permanent makeup must be light enough for us to draw hair strokes. We ask anyone with a previous eyebrow tattoo to send us an email to jealousymakeover@yahoo.com with a picture of their eyebrows with no makeup on, so we can evaluate. Cancellation Policy: POLICY ACKNOWLEDGEMENT At Jealousy Studios we ask that you give us 1 full business day notice for canceling or rescheduling an appointment. This policy enables us to schedule another guest and, in turn, maintains a higher availability for you as well as others. If you cancel your appointment and the applicable 1 full business day notice is not given, you will forfeit your booking fee. Late cancellations and missed appointments or “no shows” will not only forfeit their booking fee, but an additional $150 pre-payment of service will be required prior to rescheduling a new appointment. Please respect our time and our clients time. If you are running more than 15 minutes late, please, let us know and we will let you know if we can accommodate you. In the event we cannot, you will forfeit your deposit and a new deposit will be required to reschedule. CDC and OSHA guidelines will not allow observers in the procedure area during the procedure process. Absolutely NO children, including infants or Animals (Of any type “see CDC, OSHA & Health Department rules and regulations”) in the Studio. We uphold our right to refuse service for anyone.
